Memorial Service Planning Guide

We have
had several instances in the past within our church family when a member has
suddenly become seriously ill and we were unable to contact a family member or
responsible friend because we lacked the necessary information. Therefore,
we are preparing a form which, when completed by you, will provide information
which would be of value to the minister and the office staff in case of an
emergency. This
would include the name, address and telephone number of the relative or friend
you want to be notified and with whom we might speak about a memorial service
if needed. Guidelines are included in
which there will be space for you to provide us with ideas you might have about
a memorial service - your favorite music or readings, or information that might
be satisfying to family members. You
might want to share important life events with others. We will
have available information from the Hemlock and Neptune Societies in regard to
"Pre-need" planning and decisions long before they are needed. We will
have Living Will forms available for those who have not yet completed one. Information
concerning the placing of a plaque on the wall of the Memorial Garden will be
included. We will
let you know when the forms are available and ask for your cooperation in
